Insurance & Billing

Frequently Asked Questions

We want to make it easier to understand your insurance coverage and what you may need to pay for your care.

Does UBMD accept my insurance?

UBMD physicians participate with many health insurance plans. However, insurance participation can vary by provider, practice, location and your specific insurance plan.

Before scheduling care, we recommend confirming that your UBMD provider is in network for your specific plan.

You can also call the customer service number on the back of your insurance card and ask whether the specific UBMD physician you plan to see is in network.

What will I have to pay?

What you pay depends on your insurance plan and the services you receive. Your insurance company can tell you about your:

  • Copayment
  • Deductible
  • Coinsurance
  • In-network and out-of-network benefits
  • Prior authorization requirements

For questions about a UBMD bill, please contact the UBMD practice that provided your care.

What if I don’t have insurance, or are planning on paying out-of-pocket for my care?

If you do not have health insurance or do not plan to use insurance to pay for your care, you have the right to receive a Good Faith Estimate explaining how much your medical care is expected to cost.

You may request a Good Faith Estimate before scheduling care. When services are scheduled in advance, federal law may also require us to provide an estimate before you receive those services.

If your final bill is at least $400 more than your Good Faith Estimate, you may be able to dispute the bill through the federal patient-provider dispute resolution process.

Learn about your right to a Good Faith Estimate here.

What are my rights and protections against surprise medical bills?

Federal law protects patients from certain unexpected medical bills, including some bills for emergency services and certain services provided by out-of-network providers at in-network facilities.

These protections may limit when you can be charged more than your plan’s in-network cost-sharing amount or receive a balance bill from an out-of-network provider.

For more information about your federal rights under the No Surprises Act, visit the Centers for Medicare & Medicaid Services.

What if I have more questions about insurance or billing?

Because insurance participation and billing are handled by each UBMD practice, please contact the practice where you receive care.
